I wish to apply for membership of NWCKB. I understand the practice of kickboxing allied activities is done entirely at my own risk. I shall not hold responsible the school, its Principle, Officials, Instructors or any of my fellow members, for any injury I may sustain. All information given in this form is true, to the best of my knowledge.
